PT Health Navigator
This is a community health position in a fast-paced, non-medical environment. Responsible for providing health navigation services, health education, referral and disbursement of funds for emergency services programs. This position will research health care information and provide health outreach to clients at desk and in the lobby. Additionally, this position will play a key role in maintaining positive working relationships with partners and referral sources.  To learn more about this job, please click here.
